    The Seal of UPHSD is a cameo depicting in semi-abstract form the image of Our Mother of Perpetual help clasping in her arms the infant Jesus. This stands for the Christian precepts to which the college adheres and which binds its aspirations and hopes. The infant Jesus is symbolic of the youth who must be nurtured by a devoted and loving Mary which speaks of the motherly care and love of the college. The branches of laurel leaves signify excellence which the college pursues, not only in academics but also in leadership and in human relations. The Cameo is bordered by oval lines of gold set as maroon and white background which are the colors of the institution.     For Freshmen:
    * Two (2) 2"x2" colored ID photos (w/ white background)
    * NSO-certified Birth Certificate
    * Original Certificate of Good Moral Character
    * Original Form 138 (Report Card)

    For Transferees:
    * Two (2) 2"x2" colored ID photos (w/ white background)
    * Honorable Dismissal or Transfer Credential Certificate
    * Original and Photocopy of Summary of Scholastic Records
    * Original Certificate of Good Moral Character

    For Foreign Students:
    * Photocopy of PASSPORT (with valid stay)
    * Birth Certificate (Original or Authenticated Copy)
    * Academic Records (duly authenticated by the Embassy)
    * Duly Accomplished Application Form for Collegiate Admissions (available at the Information Center Room 135)
    * Three (3) 2"x2" colored ID photos (with white background)
    * Foreign Admissions Fee ($100 or Php 5,000.00)
    * Student Visa or Special Study Permit
